It’s been estimated that more than half of all denials for lack of medical necessity are really denials for lack of clear documentation.  Kathy understands the documentation that insurers want to see in order to paint an uncomplicated and easy to understand picture of why you’re treating the patient.  Post payment audits are being performed at a record pace.  Don’t think that just because and insurance company paid you, that you’re in the clear.  If they ask for documentation on post payment review and you can’t produce legible, unquestionable evidence of treatment, they can ask for all the money back that has been paid.  Don’t put your practice at risk. Do it right the first time, and put your head on your pillow at night confident that you can justify any treatment given to your patients.

  • This service includes, as part of a compliance review, assessment of clinical documentation of medical necessity as it applies to codes that have been charged. Comparison of daily notes to the billed codes is performed to assure that medical necessity can be proven in the event of an audit.

  • Documentation training and periodic auditing can be completed where necessary. Feel secure in knowing that another pair of eyes is helping keep you on track. 
  • We'll perform a written review of existing documentation on travel cards and other written documentation.  We will provide you with necessary templates of letters of medical necessity and documentation examples for the key codes used in the practice.
